Isn't a contract of employment a mandatory requirement to apply for a visa? Therefore, every player HAS to have a signed contract before they can even start the visa process?

I would have thought that a refusal just voids a contract, seeing as how the player would then be unable to carry out their side of it.

No it isn't. Other way round. The individual applies for the visa so if he isn't going to come under a signed contract he won't apply.

I stand corrected but my point was more that nothing is done and dusted until he runs out onto the pitch in black and white.

perfectly sensible but i tend to lean more towards getting really carried away with things and setting myself up for massive disappointment.
