I'm not suggesting he isn't accountable, but this is just players playing badly.
What's different in training to when we were over achieving last season?
What's wrong with the tactics (human error apart) both teams are set up similarly, mistakes are making the difference.
Team?? Who would have been picked differently that would have made this any different??
Substitutions - four props on the bench wouldn't alter Tim smith playing poorly or Ben Cockayne letting Dixon in for four tries or Carney running Fox and Collis ragged.
H/t talks the players should have been shamed into a better second half, if they were I've missed it!!
Seriously pick 15 better players (I'm allowing Collis and Tatuai to play next week) in our squad and don't include those that were dropped from last week.

Most of these were immortalised last season, as was the coach!!
We expect a professional performance from professional atheletes with structure in defence & attack. Our defence is absolutely woeful. We have neither structure. The coach is responsibile for this so sack the coach.
