The first column are the top eight finishers and their positions. (why 1st is at the bottom is beyond me, but it makes the rest of the table prettier)
Then you have to read across each row to see who that respective team plays at home and away.
Every number represents the finishing position of the team.
So, for example, the top row is:
Team finishing 2nd plays 3rd , 5th and 6th at home and 4th, 7th and 8th Away.
