To be fair to the Fax chairman, he sincerely apologised for it and said himself it was unacceptable. Think the programme was checked a lot more closely after that.

My favourite programme oversight was in one of our early Grand Finals, where a pen picture of Stuart Cummings described him as fat, lazy and blind. Tounge firmly in cheek of course but I believe he wasn't too happy and lodged an official complaint
