| I don't think that it was said that the closed fist made contact with the head, but it's an indication of intent. Not to punch, but to go in recklessly for a big shot. It's poor technique with no ability to wrap the tackle, similar to the shoulder charge.
He went for an aggressive shot, mistimed it, hit the head, so is automatically a red card

Not sure of the significance of the alleged clenched fist, it was only when I watched on TV that I realised it was something the two refs had brought to proceedings. FWIW I have no problem with a red for a high and hard swinging arm
